Laminated Lv Thrombus
The primary risk of lvt is the occurrence of cardiac embolism in which the thrombus detaches from the ventricular wall and travels through the circulation and blocks.
Laminated lv thrombus. A thrombus formed gradually by clotting of the blood in successive layers. Contemporary epidemiologic data suggest the incidence of lv thrombus detected using optimal imaging modalities may be as high as 15 in patients with st segment elevation mi stemi and up to 25 in patients with anterior mi. Doac use is associated with higher rates of stroke and systemic embolism than warfarin for lv thrombi in a multicenter retrospective analysis. Lv thrombus is not an uncommon complication of acute mi and is associated with systemic thromboembolism.
Acute procedural outcomes complications and clinical outcomes at 1 year were assessed. Tte is often superior to tee for this indication because lv apex is. It means not coded here. However with improvements in percutaneous coronary intervention.
Prospective studies are needed to directly compare doac and warfarin therapy for lv thrombi. Patients with laminated lv thrombus on transthoracic echocardiogram who underwent scar mediated vt ablation at two centers from 2010 to 2013 were retrospectively analyzed. Left ventricular lv thrombus is a feared complication of lv dysfunction associated with high rates of systemic embolism morbidity and mortality. Patients with documented resolution of lv thrombus by echo may still experience stroke or systemic embolism.
The most common location for lv thrombus is lv apex fig. Studies have demonstrated high incidence of lvt following anterior myocardial infarction lvt carries both short term and long term risk of embolic events which may result in stroke and systemic complications 2 3. A type 1 excludes note is a pure excludes. All patients had failed medical therapy.
Although lv thrombus is much more common with anterior mi 25 30 than in non anterior infarctions 1 5 3 the anterior location has not been shown to be important predictor of stroke in multivariate analyses. Lvt is a common complication of acute myocardial infarction ami. A type 1 excludes note indicates that the code excluded should never be used at the same time as i51 3 a type 1 excludes note is for used for when two conditions cannot occur together such as a congenital form versus an acquired form of the same condition. Left ventricular thrombus is a blood clot in the left ventricle of the heart.
